The The Ohio Department of Public Safety Division of Motor vehicles now requires student drivers to produce a driving record - 50 Hour Affidavit form to be filled out and notarized prior to turning it into the DMV. Be prepared this summer with your teenage drivers. The BMV 50-Hour Affidavit and BMV 24-Hour Affidavit are forms required by the Ohio Bureau of Motor Vehicles, most commonly for teen or new drivers applying for a driver’s license.
These affidavits confirm that the applicant has completed the required number of supervised driving hours—either 50 or 24—while accompanied by a properly licensed adult. This requirement helps ensure that new drivers have adequate hands-on experience before receiving full driving privileges.
Because BMV regulations and documentation requirements can change, it’s important to review the most current guidelines issued by the Ohio BMV. The affidavit must typically be signed by both the supervising driver and the applicant, certifying that the practice hours have been completed truthfully.
The law requires anyone under the age of 21 to complete specific driver training and practice hours before being eligible to obtain a driver’s license.

What’s required now
Applicants ages 18, 19 and 20 must complete a driver training course approved by the Ohio BMV.
The course includes 24 hours of classroom instruction and eight hours behind the wheel with a licensed driving instructor.
In addition, drivers must log 50 hours of supervised driving with a licensed driver age 21 or older, including 10 hours at night.
All practice hours must be documented using the BMV Form 5791, also known as the Fifty-Hour Affidavit, which must be signed by the supervising adult and notarized.
